Pingel also has a nice chrome heel/toe eliminator...it has a hard rubber insert to keep it from rattling, and it fits really tight.  There is a good photo with one installed on a bike on their website here:

It's about $29.00, but I found it much cheaper on ebay a few months back.

Check out www.bakercustominc.com. They have extended shifters, behind shifter spacers for
use with heel and toe or just toe shifters. I have the extended shifters and behind shift spacer
on my bike and the quality is excellent. Chrome finish is perfect. Can use all my floorboard now
and still use heel shifter. Cannot imagine better looking or fitting pieces. Highly recommend.
