can some one tell me the part number for the 2015 radio connector and the part number for my 2012 cvo antenna connectors.  I am going to need to make a short patch cable to make this work.

If I understand your request you need to plug in both coax cables on your 2012 Street Glide, to a 2015 tour pak. Best bet is to replace the tour pak cables (each less than two feet in length) with 2012 part numbers.
