69214-04 is the console assembly, fuel tank. 69215-04. Is the gasket. The insert is 71494-04bjk. Starlight black. 71495-04bkl is jet black. Those are for the 04s.

I know this is an old post but does anyone know if the insert, either jet black or starlight black, have the CVO lettering on it at the top?
Mine is peeling from the edge and want to replace it

Mine started that peeling deal also, Danny, and I tried ordering a replacement, which I was never successful in getting and over time, the rest of the clear peeled off and it looked OK again. Just give it some time. spyder
