I have a 2008 Dyna CVO. I too had all of the same issues, my feet never sliped off but I did feel them sliding to the outside of the pegs. I also had to get used to moving my right foot inward to apply the rear brake. To fix these problems I did two things. 1) I added the heal rests. (HD model style). 2) I modified my foot pegs by using a little machining, to add a small screw to act as a spacer so that both pegs now are slightly tilted inward. The angle of the foot pegs helps to keep the feet from wanting to blow to the outside of the pegs during high speeds or when a truck passes in the other direction.
