Sorry for the late reply. I just got back from the Vegas Bikefest.  

The plate and mounting hardware is available from High Gear Specialties http://www.stemstand.com/  They sell the chrome plates with different sized arms and handlebar clamps, all in chrome.  They are adjustable in a number of ways so it takes some time to get it right.  I would order it with the short & long arm in case you want to move it.  If you like I can get some better pic's of the mount for you.  I used the same product for a camera mount & sat radio mount.

The cell phone is a Motorola V600.  Hope that helps.
