Yup, it's an S&S part.  The one in that picture is for a dual runner throttle body, the attachment shows the one for a single runner throttle body.

I already checked the S&S website and they don't have this air cleaner kit.
Look carefully at the lower elbow, it is different from the S&S catalog.

The lower elbow does not look like a production piece, it looks to me like someone tried to duplicate the S&S.

Had to blow up your photo before I realized I'd seen something very similar elsewhere.  Finally found it, Wimmer is the place.
