can you adjust the air/fuel ratio with this, or is it strictly what Vance and Hines thinks it should be?

I think it does let you go in and play with the tables if you want. I strongly recommend against it unless you REALLY know what you're doing and I can guarantee you I don't. I played with mine a little but afraid to go down that road in fear of doing damage for my ignorance in that field.

Mine worked pretty good on their tune with what I have. I did the autotune and applied that to their tune, but didn't really notice any night/day difference. You have to run the autotune long enough to fully populate all the tables, they say it will be shaped like a football when you are there or close to it. It's pretty hard to fully populate every single rpm and speed and throttle position.

The only two changes I made were setting my throttle to full race (very responsive) and I changed the idle speed a little when it gets to a certain temp. just so I would know it's getting pretty hot without pulling my phone out to actually see the temp. All in all I love mine and no problems as of yet.

Absolutely LOVE it ... the looks, performance and best sounding Harley I ever had. In the past I have tried V&H  Big Shots, Rinehart True Duals, V&H Power Duals/Monster Ovals and this pipe beats all of them hands down. If you can get past the naked look on the left side of your bike you would probably love it. Keep in mind a 2-1 isn't the typical HD sound most like, it has more of a racier sound ... listen to some and you be the judge.

Check out Dragos new 2-1 vid in this same section and mine sounds real similar to that plus they also sell a ghost pipe (V&H doesn't) if you don't mind carrying around a dead pipe just for looks ... the fake thing just isn't my style.

i used to run a white bros 2:1 e-series on an older ultra so i am used to the naked left side

I was thinking like that, too. Then I gave it a little more thought and figured if I get a different system just to have a real muffler over there I will still have the same weight as the ghost, but I won't have the performance of the 2 into 1. I actually don't mind the naked left side on most bikes, just dress it up with a chrome belt guard, but the CVO's with the extended bags need a pipe or ghost on the left. I have the new Dragula from Dragos on order.
