Does anyone know at what temp and which sensors start the EITMS mode to kick in on a 2012 motor?

The engine temperature sensor, located at the left rear of the front cylinder head, and 287°F is the trigger temp.

Is that based on the temp of the oil or the metal temp?

The temperature is read from the front cylinder head. It is found on the left rear of the front head.
Engine temp and cylinder head temp are derived from this sensor.

Does the motor sound and feel like the engine is missing? I'm starting to get that more often now. it throttles up ok and is smooth. Between that and the primary rattling when clutching when hot, is starting to annoy me.

harley has always maintained that the eitms is for rider comfort, not for engine longevity.  so in that vein, if it annoys you, turn it off.  all you have to do is to roll your throttle forward and watch the cruise control icon on the tach.  if it flashes red, it is off.  flashes green, it is on.  (from 2008 on.  some early 2008 models did not have this capability)

Is this done with engine running or with ignition on and engine off?

Either way.  Originally it was with the ignition on and engine off when they first started using this system, but they quickly changed that later in the model year (2008).

Enable/Disable EITMS
1. Turn the ignition ON with the bike at idle and not moving (the bike may be running or not running).
2. Push the throttle to roll-off position and hold.
3. After approximately 3 seconds, the cruise indicator will flash either red (disabled) or green (enabled).
4. Repeat procedure to change.
