If he's done that particular build on that particular motor before, and you trust him enough to do the work to begin with, his has to be pretty good advice to factor in when spending the dollars.  Is there a reason you're doubting him?

Well if you have the heads off to mill, you might want to get them ported, clean up the intake and exhaust ports.

Honestly, yes.  If one is trying to "fix" the 110 issues all the history here seems to suggest you can't do that and keep the stock cylinders.  So if you're already willing to crack the top end one would have to at least strongly consider doing a set of Axtell (or other) cylinders and supplied mated pistons.  Then the head work is done to match whatever you've done with the slugs.

if i get the heads ported and polished what could i gain from that? if i did that would i need a bigger throttle body? im trying to get the most without spending a ton of money.

Howie might be best to speak to the throttle body question.  He got to see options in comparison speak on a dyno.  It has seemed that 110s like a larger throttle body than the stock unit though.  If you make the rest of the top end breath better that would suggest an even greater need to consider a change.

some say 190# cranking pressure is good for a street motor...that would be 11.2 with 264 cams or 10.8 with 260 cams

Might want to go a little conservative with the CR, since the stock heads don't have the greatest anti-knock characteristics (squish, flame travel, etc.) - possibly 10.5 with a stock thickness gasket? :nixweiss:   If you decide to send the heads out for some major reshaping rather than just a quick milling, then go with the recommendation of your head guy.  Another thing to think about is the amount of material you would need to remove to obtain a 2 point increase in CR; rough guesstimate .110" - .120".  Might want to look at having higher CR pistons installed instead, when they are replacing your cylinders and head gaskets for the oil leak (I'm assuming you will experience this if you haven't already).

no ploblems with mine yet. the guy who does the work for me said he never heard of the cylinder problem, but he did have some of the head gasket leaks and knows how to fix the problem

just noticed it's a fatboy, I was thinking 110" which has a better squish band on the heads...grc is right about keeping compression conservative with 103 heads...however there is the option of having the porter weld up the chambers, which would give quench and raise compression
